This hospice app provides a convenient way for healthcare providers to refer patients to Circle of Life Hospice which serves all of Northwest Arkansas. This app also provides information about clinical services provided by Circle of Life Hospice. Healthcare providers will find guidelines on hospice eligibility. Referrals submitted through the app will be received by Circle of Life’s admissions office for expedient follow up. All information received through the referral form complies with and will be processed according to HIPAA regulations.
